Apple Touch Icon not rendering as bookmark

I have an Apple touch icon included on a website that renders fine on the iPhone when selecting “Add to Home Screen”, but when selecting…

  • Add to Reading List
  • Add Bookmark
  • Add to Favorites

…no preview image gets displayed (or added).

My meta tags specify an image for “apple-touch-icon.png” and for “apple-touch-icon-180x180.png”.

Why would the preview for “Add to Home Screen” render the image properly, but not for the other 3 options above?
