Hola @aristizabalmoreno !

Parece que a tus elementos label e input les estarían faltando los atributos de for e id respectivamente.

Prueba agregarlos sobre los elementos que da el ejercicio en lugar de crear nuevamente los elementos, de esta manera evitarías repetir código también.
