This one was a little bit challenging than the first one. I had to take a look at the example quite a lot of times to figure out how each element is used and positioned;

Looks nice.

Didn’t really look much at the code but I would decrease the margin between the labels and inputs for the first three elements. Maybe also add a bit more padding to the form so everything isn’t so close to the edge.

Good job, keep it up.
