Covid 19 survey form, help me improve!

Hi guys,

I would appreciate any criticism, comments, suggestions to help me improve.
So basically, I created my own touch on the Survey forms.
I am really a newbie and I’m having a hard time applying @media queries or responsive tags/units. I don’t know the best way to get a responsive website. I also am having a hard time using the right units to make my website responsive on every device.

I had Covid-19 in January and thus it is my experience that suggests that you add hallucinating and dizziness to your list of symptoms. (It’s very common.)

I’d suggest you add a bit of UX. E.g: change the background color of submit button on hover… You can also use cursor: pointer; on that button.

Your form looks good @ckrchanuu. Some things to revisit;

  • Run your HTML code through the W3C validator.
    • Since copy/paste from codepen you can ignore the first warning and first two errors.
    • There are HTML coding errors you should address.
  • User’s should be able to click on the label to select, not just the check box
  • change the cursor to a pointer when hovering over the submit button
  • your page needs to be responsive. There’s a horizontal scrollbar on anything smaller than desktop
