Hey, nice job here, overall looks very good. Just a few things to point out:

  • Make sure every input has a <label>. This includes <select> and <texarea>.

  • For the radio button and check box groupings I suggest you use a fieldset/legend.

  • Styling issue: I think most people will find it easier to read if the check boxes were in one column instead of side-by-side. I would suggest the same for the radio buttons as well but because there are only three of them you might be able to leave it as is.
