I like your questions! I would,
give more space between select dropdown and your question.
give less space between radio and checkboxes and your questions.
give more space between textarea and submit button.
change cursor when mouse hovers on your submit button.

Use http://quirktools.com/screenfly/ to see what your webpage looks like on smaller screens.
I would suggest changing the width to 100% before 520px;
It is not as good on tablet because when you start to get to mobile is when you reach 520px;
