You seem to have added way more than was asked for. I would restart the step to get the original HTML back. Then, only add one radio button input (along with its text) right above the text input. Don’t worry about labels, just add a single input.

P.S. You should use all lowercase for tag names. Technically, you can use uppercase, but no one does and you would never use uppercase in production code (at least not for regular HTML tags).
