I am not there yet to give much advise. I can say that you might want to clean up the format of the code spaces. You can also fix the size of the form itself. Even out the border and backgrounds not the background. Try to make the page flow with each other. You have different parts of the form not even with each other. That is all I can see at the moment just woke up. lol
