What problems are you having? The things that jump out to me are that you don’t have a closing tag and you have at least one typo in your action url.

Thanks for the reply, where is the typo? and the closing tag is at the end of the code, no?

Sorry. I didn’t see that it scrolled to the side. You do have a closing tag.

I can see a random / in your url. I don’t know if it has other typos.
