Also there is no value attribute on the input tags.
Reset your code and add the value attribute for all radio and checkbox input tags.

The label tags are missing the for attribute and aren’t properly closed.
Also there is no value attribute on the input tags.
Reset your code and add the value attribute for all radio and checkbox input tags.

In your input tags, add value=“name of the of the input” to find the input name, just look before the label closing tag the name should be here /label
