The instructions are asking for the following:

“…add three fieldset elements within the form.”

Instead, you have added three divs with a label and input inside each one. There is an element called a fieldset. It has both an opening <fieldset> tag and a closing </fieldset> tag. You are supposed to add three of those inside the form.
