Hola.

Ya tienes la mayoria del trabajo, sin embargo, tienes un error en los atributos for de tus elementos label, para vincularlos con los input debes hacerlo mediante el atributo id.

Te ayuadare con el primero:

<label for="first-name"> Enter Your First Name:<input id= "first-name" /></label>
