Both the two radio buttons in the code have a name attribute :

   <input type="radio" name="account-type" /> Personal Account
   <input type="radio" name="account-type" /> Business Account

So, now what you have to do is add a name attribute for each and every submittable element in the form. Which means all inputs, textarea fields, selects …
