Give each submittable element a unique name attribute of your choosing, except for the two radio inputs.

“submittable” elements are elements in which the user can add some input, and are submitted with the form. So, pretty much all that you have there.

The name element is used to link together related inputs, for example all the elements in a checkboxes group have the same name, so keep that in mind when you give the name attribute to your various inputs!
