You need to add the name attribute, not an element here. What issue are you having?

hi there,

you’re not supposed to add any elements in this step.
Instead you’re supposed to:

give each submittable element a unique name attribute of your choosing.

So that means you should add a name attribute to the elements that are ‘submittable’ in the form.

If you’re not sure which elements this refers to, take a look at your preview pane and attempt to see which elements shown can be modified by you. Any element (other than the radio buttons) that you can change or modify, is a submittable element, and needs a name.
