.mp4 file will not play on iphone Safari or Chrome

Hello everybody! So, I am trying to play an MP4 file (.mp4) on my website. I can play it on my computer just fine with chrome, but cannot play it on my iphone through safari or or chrome (I have an iphone 5). The video has the border around it, and play button with volume controls, but is completely blank and will not play anything.

If you guys could help me get this figured out to where the video will play on my iphone too that would be great! Thank you so much for your time. The code is posted below:

CSS incase you need it for some reason

Normally it’s because the encoding is incorrect for iPhones; Google for encoding your videos to play on iPhones, I think there are some web apps that let you dump your video in and give you the correctly encoded version back, or you can use ffmpeg (any Google results pointing to Stack Overflow will normally have a few answers that include the settings you need to get the correct result out of ffmpeg).

This may not be the reason, but I would try reencoding before anything else.
