Good work! Few suggestions.

  • I would give some spaces to each different types of input sections. http://blog-en.tilda.cc/articles-website-design-mistakes
  • change mouse to cursor when it hovers on submit button.
  • Give some space under the survey form.
  • Go over syntax warnings in your html by clicking on down arrow and analyze html on codepen.
