Maybe user name, and select age between with selection list (is age important)
and experience with (list) HTML, CSS, JavaScript, building web pages, etc.

This was just a sample and my aim was to test myself in styling.
But I like your note about the privacy. It can be helpful to add a note that states that we care about the user’s privacy.
