I got hung up on putting the radio boxes and checkboxes in a separate line without using the br tag in html. Any advice with that in particular (and any other comments) would be much appreciated!

Sorry, I originally closed this post thinking it was a duplicate but you are just asking for feedback so it’s ok.
My main thing is that the form itself is a little hard to read because of the background image. So maybe you can choose a different background image or add a darkened linear gradient to the image.
