Radio button does not trigger text fields

Novice here–I’ve been trying to google my way through making some changes to a site built by another person. It’s a basic application site with yes/no questions that trigger additional fields depending on the response. Problem is the radio button isn’t triggering anything when “yes” is answered to Question 2-- I can make the fields appear when I manually change display: none to display: block, but that’s about it. Thanks in advance.

it’s hard to do this kind of behaviour with only html and css, what is your JavaScript code?

Would this be it? the radio button works correctly on the original page, but when I copy the code to use on another page it loses functionality.
