Suggestions :
comment section should be inside the form class
alignment of ratings and topping should be similar to that of name and email
remove value=name form id=name

  1. The font size does not match and the color is too dark. It is harder to read and is too small. Pick a lighter background color.

  2. Instead of putting the checkboxes in a row, try to align them properly, vertically to make it seem a little less cluttered

  3. Submit button is outside the div.
