Survey Form - Build a Survey Form

Tell us what’s happening:

Please Help. FCC tester says that I must have at least two radio buttons that are descendants of the #survey-form. I am getting the same error message for my element. I have tried everything I have seen on the forums an even tried using chat-GPT to solve this issue…to no avail.
