Your form looks good @michellebarbosa789. Some things to revisit;

- Run your HTML code through the [W3C validator](https://validator.w3.org/#validate_by_input).

- Codepen provides validators for HTML, CSS and JS. Click on the chevron in the upper right of each section and then click on the respective ‘Analyze’ link.

- A lot of the HTML code is the same as the sample. You should make the project from scratch, with your own code, style and content. Don’t take code from the sample project.

In the future when asking for feedback open your topic in the #project-feedback subforum.  
I’ve taken the liberty of moving it for you.
