Finally done with my survey form and I am delighted to finish it. Initially it looked so simple and I thought it is going to be a piece of cake but eventually I learned so much from it. Mine is pretty similar to the fCC referred sample but it took almost three days to complete. Makes me wonder how long will it take me to complete a website from scratch when I start real work.

Good job, Looks clean and easy to navigate.

One thing I can suggest is when you hover over the submit button, you can change the cursor to be something different than the default.
