I passed all the criteria for the survey form, thought i would enhance slightly on the existing tasks, click on the link and would love to receive your constructive feedback, i made the form responsive adding media queries for different browser sizes

Your project looks good and achieves the functionality.
I would recommend changing the font for the text or maybe making the font a little bigger in size.
Great job though. 
I like the colors and the border-style.
But on my chrome browser the input-fields (name, email, age) didn’t align, so the age-field ist more on the right than the other
