Survey Form (Phantom of the Opera)

Hi! I wanted to try my hand at the survey form. I based my concept on The Phantom of the Opera because I thought it would make things a little more entertaining on my end and I enjoy keeping things wacky. I don’t usually make forms so I’m happy to say that it passes the tests and validates. I’m looking forward to hearing feedback anyone might have.

The Phantom of the Opera turns to the internet to find new music students.

Your form looks good @Arrodrich. The only thing I’d suggest is to change the cursor to a pointer when hovering over the submit button.
