Survey test keeps saying I do not have two radios when I do

I have a group of radios and there are two in there. I cannot tell why that part keeps failing.

Check the name attributes of the two radio inputs. The names should be same.

Now, I need to figure out why my form is no longer centered and why the page is not actually responsive! At least I passed the test now.
