Trouble creating a form

Tell us what’s happening:
I can’t figure out how to create a form. I typed, corrected, and retyped after watching several tutorials but continue to get the same test result. "Be sure to nest your input in the form. I’m not sure what is wrong. Can somebody please take a look at my code at the bottom the page.

It is supposed to be URL not url here
