When is it best to address Media Queries in Web Design?

This is more of an efficiency question. As I am working on my Responsive Web Design projects, I find myself conflicted on when to address Media Query issues. Should I address page size issues as I add elements (ie. images, video, code, navigation bar), or should I nail down all the page content and address the the Media Query design once my page is set at its native size design?

Is there a right way or a wrong way? Is one work flow better than the other? What are the opinions of other members in the community?

I am not an expert, but I would take this approach. Before I start coding for my apps, I do a basic sketch and wireframe its outlines so design is pretty much already done (This is where I also do responsive designs thinking of each screen views). Then, I can easily structure my app using media queries.
