hi, doing the hotel feedback form and not liking my head info. not sure why. and then using a screen reader. so pasting the erorr and my code. why is it falling over at the start? Oh not a good start

Oh not a great start. will paste the error and my code. how to get this to work and to pass. maybe me being dumb, or not getting it or not able to see as totally blind.
