What error are you getting?

SyntaxError: unknown: Unterminated template. (24:97) 22 | const HTMLString = ` 23 | Entry ${entryNumber} Name</label/n> > 24 | Entry ${entryNumber} Name /> | ^ 25 | }

You’re missing your closing string symbol. It thinks everything after the input element is part of the HTMLString variable.

This is a proper string.
