You have to add value attribute in your checkbox. Your value and for attribute should be same,
For Example;
<input type = "checkbox" value="name"><label for="name">
<input type = "checkbox" value="good"><label for="good">

Hello!

Inadvertently, the value attributes and their values have been placed within the label elements, instead of the input elements.
I suggest either just moving them, or resetting the step and entering the information as you have it, but within the input elements. No changes are currently necessary to the label elements other than removing the value attributes and their values.

I have highlighted the necessary information that needs to be changed.
