The instruction says to link the form element to the label but the checkbox is saying that my first input should have id attribute.

You added all the values in the textarea and it’s label. That is wrong. You need to add it in correct label and input as a id and for attributes values.

so I have to create four label elements or just four textarea elements.

profile-picture value is goes in first label and first input elements of your fieldset. age value is goes in second label and second input.
referrer value is for select element.
And bio value is for textarea label and textarea elements. For respective id and for attributes.
