Please I am confused here.

I’d like to help as I just finished this part, but please give me more details - which part are you confused about, what’s the issue.

Give each submittable element a unique name attribute of your choosing, except for the two radio inputs.

getting confused on how to do that.

submittable element here would mean input element, and values for name attribute should be unique.
