I’m not clear what the issue is here.

There is an example in the description:

Do it like that, but make the appropriate changes.

It looks like you’ve tried to nest a form inside an input.

Your input tag should be inside the form tag.
Just like:
redacted

Please don’t just “give the answer”, especially to curriculum challenges. It’s great that you want to help, but we should be guiding people, not giving them the answer to copy.
