For the survey form, great job!
Maybe get rid of the image outside of the first one and expand the one in front.
Add a light border around your form.
For the tribute page, you did an awesome job with the new design.
The link at the end that says “wikipedia entry” is a pain on the eyes to read. change it’s color to dark blue or red.
