I’ve edited your post for readability. When you enter a code block into the forum, precede it with a line of three backticks and follow it with a line of three backticks to make easier to read. See this post to find the backtick on your keyboard. The “preformatted text” tool in the editor (</>) will also add backticks around text.

You created two input elements (both of which are incorrect) outside of your form. You are supposed to modify the first checkbox and first radio button inside your form to be checked by default.

No. The whole point is for you to find the solution. Re-read the instructions carefully to learn how to set an input element to “checked” by default. Then modify the first checkbox element and the first radio button.
