Good stuff. I’d put something behind the text and get rid of the opacity to make things easier to read. Also the form elements aren’t responsive. Easy fixes. Pretty sweet

Hi @hudsonsirois, your form looks good. One thing you should do is make the page responsive. The fields are too wide on smaller screens.
As an aside, make the submit button look like a button instead of a bar.
Nice! This is a really creative form.
I would suggest that you pick a font that matches the subject of your form.
Also, thinning the borders of your items and fixing the background image to not move would be great.
Overall, great job. Keep up the great work!
