After about 3 different designs, I finally settled on something I like (colors are a work-in-progress). I’d really appreciate your feedback on it. I’m 98% sure it’s fully responsive, but let me know if there’s something I should tweak.

make your introduction’s width the same as others below.
Enable clicking on text labels will also auto check checkboxes and radioboxes.
