I would love to get some feedback on my survey form. I’m fairly new to this but any pointers would be great.

Looks pretty good. I’d mess around with the size/alignment/spacing of your inputs. On desktop it is a bit hard to follow and on mobile there are some spots like this:

Adjusting the padding/margin etc could improve the overall usability of the form.
