Go ahead, and give each submittable element a unique name attribute of your choosing. Except for the two radio inputs.

The first hint I am getting when I use your code is:

“You should give the select element a name attribute. PS I would have chosen referrer

Have you tried doing this?

you mean i must to but (referrer) in name value ?

You added the name attribute with the value insert for this input. You would do the same thing for the select. A select is a type of form input and it can have a name attribute.
