Facebook Get quote button display Quotation form

I’ve worked very hard on the Facebook Get quote button to display Quotation form like below in image.

I try doing Edit button to right hand side and select “Get Quote” It is asking me to answer the question instead of the form option.
As i could not see any above feature, so i was wondering if anyone know how this can be done?
