Good choice i’ve done it that way too but i suggest you to make background white and text color black for better contrast, it is hard to read right now. I’ve did the opposite survey page.
Also nice font for header, very fitting

The transparent white background can be cool, but I would change the opacity a bit to make it a little bit more solid, but not too much where it’s just solid white. I would also suggest to give your form container itself a little bit of border radius, something like 3px should be good. I
