Feedback on Survey Form

I´m in the process of building the survey form. A lot of things are a mess. Right now, I´m trying to figure out how to order the checkboxes so they´re all in a column next to the corresponding textI´m posting a link to my Code Pen, which is where I´m working on it right now.

You seemed to have done it for the first group of check boxes. Can you just follow that same pattern?

Also, you have put all of the check boxes in the second grouping in one label element which is not correct. Each checkbox should have its own label.

Also, when using a fieldset, you want to use a legend for the main question instead of a label.
