Help with Form/Nesting

I don’t understand what I’m doing wrong. I followed the instructions exactly, but nothing works. I even went to the forum and tried all the other stuff people suggested and nothing worked. Is something wrong with this particular lesson? Or maybe the instructions are unclear or incorrect, because I looked at the example, and you can see for yourself that my code is exactly like that. Why doesn’t it work?
