HTML data need to be input in excel sheet

Hi team,

I have requirement to create a survey form and those user inputs need to be stored in excel sheet and it should append the data in excel sheet not override the old data.

I need survey form with good background image

can anyone help with HTML code

This is my code using PHP and HTML.

I need user input data need to store in csv file and record need to append when user add the data also please advise how can i use good background to look a good webpage.
