I’ve edited your code for readability. When you enter a code block into a forum post, please precede it with a separate line of three backticks and follow it with a separate line of three backticks to make it easier to read.

Along with the three sections the form has, include this code snippet: <button type="submit">Send</button>
, so that the code may look something like this:

<button type="submit">
Send
</button>
</form>
I think you’ve mistakenly closed the button tag with an input tag: <button type="submit">Send</input>
, and the rest seems fine.
Note:
If there’s only one button inside the “form”, then there is no need to mention its type as “submit”. Its type by default will be set to “submit”.
