Your survey form has some wonky responsiveness. :thinking:

Welcome to the forums @mccartneyj2009.
The link is not readily noticable on your tribute page. Think about users that don’t have a mouse to use to hover.

  • Run your HTML code through the W3C validator.
    • There are coding errors you need to address.
  • Users should be able to click on the labels to select, not just the radio button/checkbox
  • Make your form responsive
