Please Help Create a Form Element

I am struggling with this exercise. I am unable to pass this exercise for the following reasons.

  1. The existing input element should be nested within a form element.
  2. Your form element should have well-formed open and close tags.

where did I go wrong with my open and close tags and nesting within my form element?
