What exactly is not working? What happens when you click the link?

There is only test kit nothing else. No HTML, CSS, JS parts, just blank white page. There isn’t any console or forking button at below.

But I forked from previous link (from Build a Survey Form project). So problem solved.

I am glad you managed to get it sorted. It sounds like an issue with CodePen, though, and not the freeCodeCamp boilerplate.
